¿De qué hablamos?





¿Cuáles son los factores de riesgo?



  • Categoría 0 (riesgo muy bajo): ausencia de neuropatía periférica y de enfermedad arterial periférica.
  • Categoría 1 (riesgo bajo): presencia de neuropatía periférica o enfermedad arterial periférica.
  • Categoría 2 (riesgo moderado): presencia de neuropatía periférica y arteriopatía periférica o neuropatía periférica y deformidad del pie (dedos en garra, en martillo y artropatía de Charcot) o enfermedad arterial periférica y deformidad del pie.
  • Categoría 3 (riesgo alto): neuropatía periférica o enfermedad arterial periférica y uno más de los siguientes: antecedentes de úlcera en el pie o amputación previa o enfermedad renal en estadio final.

¿Cómo se diagnostica?




Inspección cutánea



Exploración neurológica




  • El paciente debe estar con los ojos cerrados en posición de decúbito supino.
  • Aplicar en primer lugar el monofilamento en las manos o en el interior del brazo del paciente para que el paciente comprenda el tipo de sensación que debe percibir.
  • Aplicar el monofilamento perpendicular a la superficie cutánea con fuerza suficiente como para doblarlo.
  • Preguntar al paciente:
    • ¿Siente la presión aplicada? (sí/no).
    • ¿Dónde siente la presión? (pie izquierdo/pie derecho).
  • Debe aplicarse el monofilamento en el perímetro del sitio de la úlcera, no sobre la úlcera.
  • No permitir que el monofilamento se deslice por la piel o que contacte varias veces en el punto de prueba.
  • La duración total del movimiento (contacto con la piel y retirada del monofilamento) debe ser de unos 2 segundos.







  • Ipswich Touch Test (IpTT): consiste en que el explorador toca suavemente las puntas de los dedos 1º, 3º y 5º de cada pie estando el paciente con los ojos cerrados. Ha mostrado concordancia casi perfecta al compararlo con el test del monofilamento 10 g. Este método es simple, rápido, fiable y puede ser usado en el domicilio por personal no sanitario (Rayman G, 2011; Zhao N, 2021).
  • Vibratip: dispositivo de bolsillo para la exploración de la sensibilidad vibratoria que ha demostrado concordancia con otros test para predecir úlceras (Boulton AJ, 2013).

  • Grado:
    • Grado 0: lesión preulcerosa o postulcerosa completamente epitelizada.
    • Grado 1: herida superficial que no afecta a tendón, cápsula o hueso.
    • Grado 2: herida que afecta a tendón o cápsula.
    • Grado 3: herida que afecta a hueso o articulación.
  • Estadio:
    • A: no infectada.
    • B: infectada.
    • C: isquémica.
    • D: infectada e isquémica.
